(McNulty, Nova Scotia Hospital)<\/span><\/figcaption><\/figure>\n<p>La th\u00e9rapie par le travail fut mise en place d\u00e8s les d\u00e9buts de l\u2019H\u00f4pital de Nouvelle-\u00c9cosse, en commen\u00e7ant par la ferme de l\u2019institution et en rassemblant progressivement de nombreuses corv\u00e9es de base supervis\u00e9es.<\/p>\n<p>L\u2019ergoth\u00e9rapie n\u2019\u00e9tait pas enregistr\u00e9e dans les rapports annuels de l\u2019H\u00f4pital de Nouvelle-\u00c9cosse avant 1950, m\u00eame si elle fut mise en place au moins dix ans plus t\u00f4t. En 1950, le d\u00e9partement est reconnu au d\u00e9but sous la cat\u00e9gorie \u00abDivertissement des patients\u00bb \u00e9tant donn\u00e9 qu\u2019il facilitait la projection de films. \u00c0 partir de 1952, le d\u00e9partement \u00e9tait responsable de l\u2019administration d\u2019une biblioth\u00e8que grandissante et d\u2019une s\u00e9rie d\u2019activit\u00e9s de loisirs durant l\u2019ann\u00e9e.<\/p>\n<p>D\u00e8s 1952, le d\u00e9partement d\u2019ergoth\u00e9rapie donna aux patients le choix d\u2019apprendre des m\u00e9tiers\u00a0: la couture, le tissage, le travail de cuir, la fabrication de gants, de panneaux, de tapis et de jouets, le tricot, la dactylographie, les n\u0153uds, le crochet, l\u2019artisanat \u00e0 partir de coquillage ou de li\u00e8ge, le tressage, la peinture et la menuiserie. Les patients masculins avaient \u00e9galement l\u2019opportunit\u00e9 de participer \u00e0 des projets de travail, aussi bien \u00e0 l\u2019int\u00e9rieur qu\u2019\u00e0 l\u2019ext\u00e9rieur de l\u2019institution. En 1959, on commen\u00e7a \u00e0 offrir aux patientes des possibilit\u00e9s de travail au sein de l\u2019institution. En 1960, le rapport annuel mentionne des efforts suppl\u00e9mentaires pour trouver d\u2019avantage de places pour les femmes au sein de l\u2019institution.<\/p>\n<p>En 1962, le d\u00e9partement d\u2019ergoth\u00e9rapie commen\u00e7a \u00e0 se tourner vers la \u00abth\u00e9rapie industrielle\u00bb. Sous ce terme g\u00e9n\u00e9ral, l\u2019h\u00f4pital pr\u00e9tendait avoir \u00abcr\u00e9\u00e9 plus de cent postes pour les patients, utilisant l\u2019h\u00f4pital comme industrie\u00bb et anticipa la mise en \u0153uvre d\u2019une \u00abpseudo-\u00e9chelle de salaire pour les patients salari\u00e9s\u00bb. M\u00eame si les patients s\u2019occupaient de la plupart des t\u00e2ches au sein de l\u2019h\u00f4pital, il ne fut jamais question de les r\u00e9mun\u00e9rer. En 1962, une proposition en ce sens fut avanc\u00e9e, sans toutefois \u00eatre adopt\u00e9e. (Department of Public Health, Nova Scotia Hospital Report, 1962).<\/p>\n<p>En 1974, le d\u00e9partement d\u2019ergoth\u00e9rapie fusionna avec les Services de b\u00e9n\u00e9voles et fut rebaptis\u00e9 \u00abD\u00e9partement de th\u00e9rapie de soutien\u00bb. Ce nouveau d\u00e9partement pla\u00e7a 192 patients dans des \u00absituations de travail au sein de l\u2019h\u00f4pital\u00bb et ces patients travaillaient 5.684 jours au cours de l\u2019ann\u00e9e. Pendant le mois de novembre 1974, un \u00abplan de r\u00e9mun\u00e9ration exp\u00e9rimental\u00bb r\u00e9compensait financi\u00e8rement les patients pour leur travail selon le \u00abtemps travaill\u00e9 et le niveau fonctionnel atteint\u00bb (Department of Public Health, Nova Scotia Hospital Report, 1974). Le D\u00e9partement rapporta que la mise en place de r\u00e9compense permettait aux patients d\u2019 \u00ab[appr\u00e9cier] leur r\u00e9tablissement\u00bb, ainsi que de reconnaitre les niveaux de performance \u00abinsatisfaisants\u00bb. L\u2019administration d\u00e9clara qu\u2019elle \u00abesp\u00e9rait que ce genre de programme pourrait faire partie de [son] service de r\u00e9habilitation au travail\u00bb. (McNulty, Nova Scotia Hospital)<\/span><\/figcaption><\/figure>\n<p>L\u2019ergoth\u00e9rapie joua un r\u00f4le important dans la transition du patient \u00e0 la personne \u00e0 l\u2019H\u00f4pital de Nouvelle-\u00c9cosse. Les privil\u00e8ges des patients \u00e9taient divis\u00e9s en fonction du genre\u00a0: les patients masculins pouvaient aller au th\u00e9\u00e2tre et aux spectacles saisonniers et faire des tour de bus pendant de nombreuses ann\u00e9es, alors que les patientes durent attendre beaucoup plus longtemps pour b\u00e9n\u00e9ficier des m\u00eames droits. Dans les ann\u00e9es 1950, les patients pouvaient aller au th\u00e9\u00e2tre de Mayfair, aux Ice Cycles (un spectacle annuel au Forum d\u2019Halifax) et d\u2019autres voyages dans la province. Plus largement et au sein de la population masculine, on trouvait des hi\u00e9rarchies de libert\u00e9s et de responsabilit\u00e9s\u00a0: un groupe de patients avait le droit de travailler dans ses propres secteurs, parmi lesquels le magasin de l\u2019h\u00f4pital et la cuisine.<\/p>\n<p>La Loi sur les nouveaux h\u00f4pitaux (New Hospitals Act) de 1979 constitua la premi\u00e8re reconnaissance gouvernementale des droits des patients. (McNulty, Nova Scotia Hospital)<\/span><\/figcaption><\/figure>\n<p>Le personnel de l\u2019H\u00f4pital de Nouvelle-\u00c9cosse \u00e9tait structur\u00e9 hi\u00e9rarchiquement: le directeur exer\u00e7ait son autorit\u00e9 sur le directeur assistant, ainsi que sur les m\u00e9decins, infirmiers, th\u00e9rapeutes pr\u00e9pos\u00e9s et ergoth\u00e9rapeutes. Alors que le nombre de patients ne cessa d\u2019augmenter au cours des deux premiers tiers du vingti\u00e8me si\u00e8cle, on embaucha des infirmiers et des pr\u00e9pos\u00e9s en cons\u00e9quence. Le nombre limit\u00e9 d\u2019ergoth\u00e9rapeutes demeura une pr\u00e9occupation constante. En 1958, le d\u00e9partement fit \u00e9tat de r\u00e9ductions dans les programmes en raison du manque d\u2019effectifs. Allant \u00e0 l\u2019encontre des tendances observ\u00e9es dans les complexes psychiatriques r\u00e9sidentiels ailleurs au Canada \u00e0 la fin des ann\u00e9es 1950 et au d\u00e9but des ann\u00e9es 1960, le d\u00e9partement d\u2019ergoth\u00e9rapie de l\u2019H\u00f4pital de Nouvelle-\u00c9cosse fut progressivement abandonn\u00e9. Ses programmes ainsi que ses employ\u00e9s furent de plus en plus transf\u00e9r\u00e9s vers d\u2019autres d\u00e9partements. Dans les ann\u00e9es 1960, le d\u00e9partement d\u2019ergoth\u00e9rapie fait \u00e9tat de manque de personnel et pendant une bonne partie de 1977, le poste de chef fut vacant.<\/p>\n<p>Avec la mise en application de la Loi sur les nouveaux h\u00f4pitaux, le 1er avril 1979, les services de r\u00e9f\u00e9rences et de garde s\u2019en trouv\u00e8rent affect\u00e9s, entra\u00eenant une augmentation des demandes de la part des psychiatres et des m\u00e9decins, ce qui, selon le rapport annuel, vint \u00e0 restreindre la dur\u00e9e de traitement direct des patients.
